Treatment: We may use and disclose your PHI to provide, coordinate, or manage your mental health care.
Payment: This is a self-pay practice. We do not bill insurance. PHI may be used for administrative purposes.
Health Care Operations: PHI may be used for scheduling, record keeping, and administrative operations.

Telehealth & Electronic Communication
Services are provided via HIPAA-compliant telehealth platforms, including the SimplePractice secure client portal. You may communicate with us via the secure portal, email, or text messaging. Electronic communication carries privacy risks, and by choosing these methods you acknowledge and accept those risks.

Minors
When services are provided to minors, a parent or legal guardian must consent to treatment. Parents or legal guardians may have rights to access records except as limited by Florida law. Confidentiality may be limited by safety and legal requirements.

Disclosures Required by Law
We may disclose PHI without your authorization when required by law, including mandatory reporting of abuse or neglect, serious threats to health or safety, court orders, or public health requirements.

Your Rights
You have the right to inspect or obtain a copy of your health records, request corrections, request restrictions on certain uses or disclosures, request confidential communications, and receive a copy of this Notice upon request.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with Julia Cole Therapy LLC or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
